ABSTRACT
Objective:To study the cause of 1 PICC catheter rupture, and discuss the nursing interventions.Methods:Comprehensive analysis was carried out on the review of the use and maintenance of PICC catheter, patient's life style and catheter quality test.Results:The main reasons of PICC catheter rupture were the pinch-off syndrome and catheter plug caused by gaming disorders and non-canonical maintenance of PICC outhospital.Conclusions:With the growing of video game players, gaming disorder may be one acquired factor of PICC catheter rupture. To reduce the occurrence of PICC catheter rupture, prompt identification of and timely intervention to gaming disorder and intensified outhospital care are paramount. The predictive diagnostic tests of catheter ectopia based on clinical syndromes, physical signs and catheter state, are helpful for the early recognition and treatment of PICC catheter rupture, followed by the risk reduction of pulmonary embolism.
ABSTRACT
Objective To invetigate the clinical effect of Guyouling Liniment in the treatment of soft tissue contusion.Methods Sixty-three cases of soft tissue contusion in our hospital from October 2012 to January 2014 were selected and divided into the treatment group (n=32)and control group(n=31).The treatment group were treated with Guyouling Liniment and the control group adopted Indomethacin Liniment.The effects were compared between the two groups.Results Twenty-six cases(81.13％) in the treatment group were cured and 30 cases (93.75％) were effective;23 cases(74.19％) in the control group were cured and 27 cases (87.10％) were effective.The clinical effect of the treatment group was better than that of the control group with statistical difference(P＜0.05).Compared with the control group,the pain degree and stasis purple area in the treatment group were significantly alleviated,the difference was statistically significant(P＜0.05).Conclusion Guyouling Liniment has significant effect in the treatment of soft tissue contusion and is worthy of clinical promotion.

OBJECTIVE:To discuss clinical efficacy and safety of leuprolide acetate in the treatment of endometriosis. METH-ODS:66 endometriosis patients were divided into observation group(37 cases)and control group(29 cases)according to random number table method. Observation group was treated with Leuprolide acetate microsphere for injection,3.75 mg/time,on the upper arm,abdomen or buttocks since the first-fifth day of menstrual cycle(administration time was available at hysterectomized patients disposal),and then every 4 weeks;one administration was recognized as a treatment course. Control group was treated with Ethi-nyl estradiol cyproterone tablet orally,1 tablets per day for 3 weeks,one week drug withdrawal,as a treatment course. 2 groups were given 3-5 courses of treatment according to their tolerance or the improvement of clinical symptoms. The improvement of clini-cal symptoms were compared between 2 groups,and the levels of FSH,LH and E2 were compared before and after treatment;the occurrence of ADR and the time of the return of menses were recorded. RESULTS:2 patients in observation group and 1 patient in control group withdrew from the test. After treatment,total effective rate was 97.14% in observation group,which was significant-ly higher than 75.00% in control group,with statistical significance (P0.05). After treatment,the levels of FSH,LH and E2 were de-creased significantly,and the observation group was more significant than the control group,with statistical significance (P0.05). The time of the return of menses in observation group was (89.75 ± 3.34) d after drug withdrawal,com-pared to control group [(88.46±2.94)d],there was no statistical significance(P>0.05). CONCLUSIONS:Leuprolide acetate can effectively improve clinical symptoms of patients with endometriosis,and reduce the levels of FSH,LH and E2 with slight ADR, and it doesn’t influence the time of the return of menses.

OBJECTIVE:To provide reference for rational use of drugs for gastrointestinal ulcer. METHODS:The drug use for gastrointestinal ulcer in outpatient department of our hospital during 2013-2015 was analyzed retrospectively in respects of drug name,consumption sum,DDDs,DDC,single/multiple diseases prescription,therapeutic efficacy,rationality of presoription,etc. RESULTS:A total of 8 kinds of drugs use for gastrointestinal ulcer in outpatient department of our hospital during 2013-2015, among which the consumption sum and DDDs of Omeprazole capsule and Rabeprazole capsule increased rapidly,and their DDC were the higher others;those of other drugs kept stable. The proportion of acid-inhibitory drugs in single disease prescriptions was lower than in multiple diseases prescriptions,while that of antiacid was higher than in multiple diseases prescriptions,with statisti-cal significance (P<0.05). The total effective rate of patients with gastrointestinal ulcer gradually increased from 77.98% to 82.68% in outpatient department of our hospital during 2013-2015,there was statistical significance between 2015 and 2013(P<0.05);the proportion of irrational prescriptions decreased from 5.01% to 2.12% year by year,with statistical significance among those years (P<0.05). CONCLUSIONS:Omeprazole is the first choice for gastrointestinal ulcer in outpatient department of our hospital with good therapeutic efficacy. There still are some irrational drug use in prescriptions,and clinicians should pay attention to those situations.

Objective To evaluate the value of technetium-99m methoxy isobutyl isonitrile (99Tcm-MIBI)tomography and integrated CT in preoperative localization diagnosis of secondary hyperparathyroidism (SHPT).Methods 28 patients with SHPT were selected.15 minutes and 90 minutes (delayed images)after intravenous administration of 99Tcm-MIBI 370 MBq,static anterior planar images of the neck and chest were obtained.99Tcm-MIBI tomography and integrated CT scan were acquired after the delayed imaging.The imaging findings were compared to the pathological results.Results The detection rate of SHPT lesions by 99Tcm-MIBI dua-phase imaging and 99Tcm-MIBI tomography and registration with integrated CT scan was 53.3％ and 70.7％ respectively.The difference had statistical significance(X2 =5.903,P＜0.05).The fusion imaging with integrated CT can provide detailed anatomy data and more information for surgical doctors.Conclusion 99Tcm-MIBI tomography and registration with integrated CT scan can improve the detection rate of SHPT compared with 99Tcm-MIBI dua-phase imaging,provide more detailed anatomy data,and has high clinical value in preoperative localization and diagnosis of SHPT.

[ABSTRACT]OBJECTIVETo study of the relationship of age and climate with the nasal mucosa lesions.METHODSAll the patients met with the inclusive criteria were registered and followed-up once a month and their nasal mucosal lesions were observed through nasal speculum and/or nasal endoscope. The recording data was analyzed by SPSS17.0 software.RESULTSA total of 259 patients, 555 person-times and 957 lesion spots were observed. The nasal mucosa was injured by airflow with the highest incidence rate from October to next year April. Patients numbers gradually increased after August, and significantly increased at November, December (P<0.05). Children aged from 2 to 12 years old and the adults aged from 40 to 80 years old were susceptible population (P<0.05), the incidence rate had a negative relation with humidity (P<0.05). The susceptible spots of the lesion were ranked as follows: Little area, inferior turbinate, mucosa of the both side of the deviated nasal septum, inferior meatus, middle meatus, middle turbinate and olfactory area. CONCLUSIONNasal mucosal injury relates to age and seasons. The nasal cavity airflow may be a factor of these lesions.

Objective To translate the Personal Diabetes Questionnaire(PDQ),and test the relia bility and validity of the Chinese version of PDQ.Methods Through forward translation,back translation,cultural adjustment,etc.,the Personal Diabetes Questionnaire (PDQ) was introduced to localize it.60 eligible patients with diabetes were selected in a tertiary hospital to make a preliminary investigation.The results were statistically analyzed in order to assess the reliability and validity of the Chinese version of PDQ.Results The Chinese version of PDQ had good reliability,whose total internal consistency coefficient was 0.831 and test-retest reliability was 0.801.The questionnaire has good validity.Total Content Validity Index was 0.938.I-CVI was between 0.800 and 1.000.The questionnaire's dimensions had good parallel validity with glycosylated hemoglobin (HbAlc) and body mass index (BMI).Conclusions The Chinese Version of the personal diabetes questionnaire has good reliability and validity,which can be widely used as the evaluation tool of diabetes self-management education.

However , they were both higher than ultrasound scan which had a detection rate of 64.5%.The differences had statistical significance (χ2 =4.039 and 7.884 respectively , P<0.05 ) .The detection rate of parathyroid hyper-plasia of 99 Tcm-MIBI tomography and registration with integrated CT was both 80.0%, higher than that of the ul-trasound test(40.0%)and 99Tcm-MIBI dual-phase imaging(40.0%).The fusion imaging with integrated CT scan provided detailed anatomy data and more information for surgical doctors .Conclusions 99 Tcm-MIBI tomography and registration with integrated CT scan can improve the detection rate of PHPT compared with 99 Tcm-MIBI dual-phase imaging , provide detailed anatomy data , and has high clinical value in preoperative localization diagnosis of PHPT.

ObjectiveTo explore the best time for replacing the long-term indwelling catheter in patients with silicone catheter.MethodsDomestic literature were retrieved and analyzed using Cochrane systematic review,and the meta-analysis of randomized controlled trials was conducted in accordance with inclusion criteria and exclusion criteria so as to look into the most suitable time for replacing the indwelling catheter for patients with silicone catheter at clinical trials.The replacement frequency for the catheters of different size together with the relevant infections in urinary tract infection and relative risk(RR)were used as values for effectiveness of interventions.ResultsA total of 11 literature were retrieved. Meta-analysis results suggested that the RR values of urinary tract infections when the catheters were replaced once every two weeks vs. every four weeks,and once every three weeks vs. every four weeks were 0.51[95% CI(0.40,0.66),P <0.001],0.79[95% CI(0.58,1.08),P = 0.14],respectively.The urinary infection rate of replacing a silicone cathether every 2 weeks was higher than that of every 4 weeks,but there was no difference between that of every 3 weeks and 4 weeks.Conclusion According to the nature of silicone catheter material as well as the clinical indexes, it is most reliable to replace a silicone catheter every four weeks to reach a best clinical outcome.

Objective To detect the expression of epidermal growth factor receptor (EGFR) and cyclooxygenase-2(COX-2 ) in laryngeal squamous cell carcinoma and to investigate their clinical significance.Methods Resected tissue of laryngeal squamous cell carcinoma (observation group)and adjacent normal laryngeal mucosa ( control group) from 54 patients were collectod.Accooding to histological differentiation degreo,cervical lymph node metastasis,clinical TNM staging,EGFR and COX-2 were compared.Results The expression rate of EGFR in observation group [63.0％ (34/54)] was higher than that in control group [42.6％ (23/54)] (P ＜ 0.05 ).And COX-2 in observation group [68.5％(37/54)] was higher than that in control group [35.2％ (19/54)] (P ＜0.01).The expression of EGFR and COX-2 in observation group (0.584 ±0.136,0.561 ±0.134) were significantly higher than those in control group (0.161 ±0.045,0.114 ±0.027),respectively (P ＜0.01).The expression of EGFR and COX-2 had significant difference among histological differentiation degree,with or without cervical lymph node metastasis,different clinical TNM staging (P ＜ 0.05 or ＜ 0.01 ).Correlation analysis showed that the expression of EGFR and COX-2 in laryngeal squamous cell carcinoma had positive correlation (r =0.602,P ＜ 0.01 ).Conclusions The expression of EGFR and COX-2 have some contribution to the incidence and development of laryngeal squamots cell carcinoma.They can be used for the different diagnosis of benign and malignant carcinoma,clinical staging and prognosis estimating.They have significant clinical values.

Objective To investigate the effect of dendritic cells(DC)pulsed with tumor lysate antigens of K562/A02 co-cultured with cytokine induced killer(CIK)cells on cytotoxicity against multidrug resistant(MDR)leukemia K562/A02 cells with high expression of p-glycoprotein(P-gp).Methods Bone marrow mononuclear cells isolated from healthy adult donors were induced to obtain CIK cells and DC respectively and then CIK were co-cultured with DCs which were pulsed with tumor lysate.The morphologic features were observed with optical microscopy and expressions of markers typical were measured using flow cytometry.The cytotoxicity of three effect cells was measured with MTT assay respectively.Results After pulsing DCs with tumor lysate antigens co-cultured CIK or DC-CIK led to a significant increase of lymphocyte proliferation of both populations.The cytotoxicity of CIK co-culturing with DCs pulsed with tumor lysate antigens to K562/A02 and K562 cells was(42.90?0.67)%、(49.85?0.28)%(63.36?0.46)% and(23.56 ?0.43)%、(26.11?0.34)%、(34.46?0.35)% respectively at an effector-to-target ratio of(5∶1、 10∶1 、20∶1).Conclusion This study indicates that DC pulsed with tumor lysate antigens co-cultured with CIK can improve lymphocyte proliferation and enhance the cytotoxic activity.Especially,it can induce a P-gp specific anti-leukemic immune.

OBJECTIVE:To observe the other effects of cimetidine in the decompensated stage of chronic cardiopulmonary disease. METHODS: 72 patients with uncompensated chronic cardiopulmonary disease were assigned into therapy group and control group (n=36). Both groups were given routine treatment. Therapy group were additionally treated with cimetidine intravenously for 48 h. The changes of respiration frequency, arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2), intake of staple food and electrolytes in blood serum of 2 groups before and after treatment were determined. RESULTS:In therapy group, the respiration frequency, the level of PaCO2 and disorder of electrolytes in blood serum were decreased and the level of PaO2 and intake of staple food were increased. Objective:To investigate whether ghrelin, a novel endogenous ligand of growth hormone secretagogue receptor ( GHS-R) , was expressed in the thyroid tissues of different thyroid diseases, and its implication. Methods:The paraffin-embedded specimens of thyroid tissues from 2000 to 2004 were obtained from 57 patients with different thyroid diseases, including 5 subacute thyroiditis, 8 Hashimoto' s thyroiditis, 7 hyperthyroidism (Graves disease) , 8 nodular goiter, 5 thyroid adenoma, 3 thyroid lympho-ma, 8 papillary carcinoma, 3 follicular carcinoma, 5 medullary carcinoma and 5 undifferentiated carcinoma cases. The specimens of normal peri-adenoma thyroid tissues served as controls. Immunohistochemi-cal staining was used to detect ghrelin expression. Results:(1) ghrelin expression was undetectable in the thyroid tissues of normal control, subacute thyroiditis, Hashimoto' s thyroiditis and Graves disease. (2) ghrelin expression was also undetected in the tissues of nodular goiter, thyroid adenoma and thyroid lymphoma. (3 ) ghrelin-positive staining was found in the tumor cells of different types of thyroid carcin-moma. Five cases were positive within 8 cases in papillary carcinoma, 2 cases were positive within 3 cases in follicular carcinoma, 3 cases were positive within 5 cases in medullary carcinoma, 3 cases were positive within 5 cases in undifferentiated carcinoma. Conclusion:Ghrelin is expressed in malignant epithelial thyroid neoplasmas, but not in autoimmune or inflammatory thyroid diseases and benign nodular thyroid diseases. The results indicate that ghrelin expression may play an important role in the occurrence and development of thyroid carcinoma.

Objective To investigate the intervention effect of Feikang Granules (FG) on airway remodeling and D-xylose excretory rate in pulmonosplenic asthenia rats with chronic obstructive pulmonary disease (COPD). Methods SPF SD rats were equally randomized into four groups:normal control group,COPD model control group,high-dose FG group,and low-dose FG group. The pulmonosplenic asthenia rats models with COPD were established by the way of smoking with cigarettes,repeated infusion of lipopolysaccharide in the airway and feeding with decoction of Folium Sennae.The general health state of rats was observed,and pathological changes of lung tissue were examined by the optical microscope. The wall thickness of small airway in rats was measured and the excretory rate of D-xylose was detected by bromoaniline method. Results Compared with the model control group,weight was increased,the pathological changes of pulmonary alveoli andbronchiole were relieved,small airway wall thickness became thin,and D-xylose excretory rate increased in high-dose and low-dose FG groups and the normal control group,the difference being significant (P
